Make A Referral
CCWJC case management staff has begun to see patients in area hospitals and assist in the planning for the transition to home. We are currently working at the WakeMed New Bern Avenue campus and remotely with Rex Hospital. We begin the planning process with the discharge planner and patient in the hospital, meet the patient as an inpatient and plan for a follow up visit within a few days of discharge. At this visit we have the copy of the discharge instructions and make sure the patient is linked to services, has filled all prescriptions and is linked back to their Primary Care Provider.
We always welcome referrals on any of your Carolina Access II patients. Any patient who would benefit from additional patient education, disease management (particularly asthma, diabetes, congestive heart failure), discussion about medication or dietary compliance should be referred to a case manager for intervention.
